Developed to address limitations of existing ophthalmic questionnaires for individuals with refractive error (including those undergoing or having had refractive surgery), this checklist provides a targeted, patient-centered measure of vision-related quality of life.

Developed from the review of published questionnaires, guided focus groups with eye care professionals and persons with refractive error, and unstructured interviews, the instrument’s structure and administration are designed for clinical and research use, facilitating assessment of symptoms, functional impact, and expectations that may change with correction method or surgical intervention.

Work presented at the ARVO annual meeting (1998) and published by the American Academy of Ophthalmology (2000).


Questionnaire structure and administration

  • 42 items organized into eight subscales:
    • Functioning
    • Driving
    • Concern
    • Expectations
    • Symptoms
    • Glare
    • Optical problems
    • Problems with corrective lenses
  • Items are short declarative statements prefaced with “Because of my vision” and “In the past month.”
  • Response formats assess either degree of difficulty/bother (not at all, mild, moderate, severe, so bad that I don’t use this type of correction) or frequency (never, rarely, sometimes, often, all the time).
  • “Not applicable” responses are permitted.
  • Items account for type of corrective lenses when addressing satisfaction, ratings of vision, and visual symptoms.
  • An overall RSVP scale score and eight subscale scores are calculated
